To file a claim with the District, please complete and submit the form below.  For questions or more information about filing a claim, please contact the Risk Management Supervisor at (760) 744-0460 or by e-mail (daviles@vwd.org).  Hard-copy claim forms are also available at the District’s Customer Service Counter during office hours at 201 Vallecitos de Oro in San Marcos. 

Hard copy claim forms may be submitted at District offices or by signing the actual printed form, scanning it, and submitting it electronically by e-mail (daviles@vwd.org). 

Amendments to a claim or applications for leave to present a late claim may be filed electronically by e-mail.  If filed electronically, the amendment to a claim or application for leave to present a late claim shall be signed by signing the actual printed document, scanning it, and submitting it by e-mail (daviles@vwd.org).
